Please read each section carefully, enter your initials in the box provided, then sign and submit at the bottom. This form is required before your first coaching session begins.

Nature of Coaching Services

Grief and life coaching is a forward-focused, collaborative process designed to help you navigate loss, rediscover your sense of purpose, and build a meaningful path forward. As your coach, Felicia Shephard provides compassionate guidance, practical tools, and a safe space to process your experience — while keeping your eyes fixed on healing and hope.

Coaching is not therapy, counseling, psychotherapy, psychiatric care, or any form of licensed mental health treatment. It is not a substitute for medical advice, legal counsel, or crisis intervention. The coaching relationship does not establish a therapist-client or doctor-patient relationship, and no diagnosis of any kind will be made.

I understand that coaching is not therapy or medical treatment and does not replace licensed professional care. I acknowledge the forward-focused nature of grief and life coaching as described above.

Professional Credentials and Scope

Felicia Shephard is a Certified Grief Coach and Certified Life Coach, and a Licensed Minister in the State of Alabama. She does not hold a license in medicine, psychology, social work, or professional counseling. She does not prescribe medication, provide clinical diagnosis, or offer any service that requires a state-issued healthcare or mental health license.

All services provided by From Grief to Grace LLC are coaching services only. If concerns arise that fall outside the scope of coaching, you may be encouraged to seek support from a licensed professional.

I understand Felicia Shephard's credentials and the scope of services offered. I acknowledge that coaching does not include clinical diagnosis, therapy, or medical treatment of any kind.

Client Responsibility

I understand that coaching is a collaborative process and that I am an active participant in my own healing. I am solely responsible for the decisions I make, the actions I take, and the outcomes of my life before, during, and after our work together.

I acknowledge that no guarantees or promises have been made regarding specific results. I agree to take responsibility for seeking appropriate professional support — medical, psychological, legal, or otherwise — whenever my needs extend beyond the scope of coaching.

I accept full responsibility for my own decisions and well-being and understand that coaching does not guarantee specific outcomes.

Emotional Risk Acknowledgment

I understand that grief and life coaching may involve discussions of loss, pain, identity, relationships, guilt, anger, and other deeply personal experiences. These conversations may bring up strong emotions, and I acknowledge this is often part of the healing process.

I acknowledge that I may occasionally feel emotionally activated, fatigued, or temporarily distressed as a result of our work together. I agree to take responsibility for my own emotional well-being between sessions and to seek professional help if I feel overwhelmed or unsafe.

I acknowledge the emotional nature of grief coaching and agree to seek immediate professional support if I experience a crisis or feel unsafe at any time.

Mental Health and Medical Disclaimer

I understand that coaching services provided by From Grief to Grace LLC are not designed to diagnose, treat, or cure any mental health condition, medical condition, or psychological disorder — including but not limited to clinical depression, anxiety disorders, PTSD, suicidal ideation, self-harm, psychosis, or substance use disorders.

I agree to release From Grief to Grace LLC and Felicia Shephard from any liability related to emotional distress, mental health outcomes, or decisions made as a result of coaching sessions. I understand it is my responsibility to seek licensed clinical care for any condition requiring it.

I understand that coaching is not a substitute for mental health or medical treatment and I release From Grief to Grace LLC from liability related to coaching outcomes.

Emergency and Crisis Limitation

I understand that From Grief to Grace LLC is not a crisis service and that Felicia Shephard is not available for emergency response or 24-hour support. If I am in immediate danger or experiencing a mental health emergency, I agree to contact emergency services or a crisis line immediately.

Confidentiality

I understand that what I share in coaching sessions will be held in confidence to the extent permitted by law. I acknowledge that confidentiality may be limited in situations involving court orders, suspected abuse or neglect, or a reasonable belief that I am at serious risk of harm to myself or others.

I understand that coaching confidentiality does not carry the same legal privilege protections as licensed therapy or attorney-client relationships.

I understand the scope and limitations of confidentiality in the coaching relationship as described above.

Communication and Boundaries

I understand that my scheduled coaching sessions are the primary space for our work together. I agree that extended coaching conversations or emotional support outside of scheduled sessions is not included unless explicitly agreed upon in writing.

I understand that Felicia Shephard is my coach and is not an on-call crisis responder or 24-hour support line. I agree to honor the boundaries of our coaching relationship.

I agree to honor the communication boundaries of the coaching relationship and understand that crisis support is not available through this service.

Video Recording Consent & Acknowledgement

From Grief to Grace LLC may conduct coaching sessions via video conference (such as Zoom or similar platforms). Please indicate your consent for each of the following by checking the applicable box(es).

I have read and understand the video recording consent options above. I acknowledge that checking the optional marketing box is entirely voluntary and that my coaching services will not be affected by my choice. I understand I may withdraw video consent at any time by notifying Felicia Shephard in writing.

By completing the fields below and checking the acknowledgement box, you are providing your electronic signature and confirming that you have read, understood, and agree to all sections of this consent and disclosure form. This electronic signature carries the same legal weight as a handwritten signature.
